The ProAct Prescription Discount Card Program was created to address the health care needs of uninsured and underinsured residents across the United States.

Participants using the ProAct Prescription Drug Discount Card Program can receive some level of financial relief on their Prescriptions, Vision, LASIK, and Hearing Aid services.

·   Prescription:      10% to 20% off on Brands and 20% to 70% off on Generics. More Info
·   Vision:                  20% to 50% off on services including: Eye Exams, Frames, Lenses and

·    LASIK:                  40% to 50% off the national average. More Info
·    Hearing Aid:       35% to 65% off brand name hearing aids and products. More Info

There are no prerequisites to utilize the discount card, anyone in the county is eligible to use the program and receive savings. Simply present the discount card at your local pharmacy and receive the discounts immediately.

• go to your local pharmacy or 
• contact the Chautauqua County Executive’s Office, Department of Health and Human Services, and Office for the Aging.
